Patient Forms
1. Three forms are listed to the left (the intake form, the patient request for confidential
communication, and the privacy practices form.) Forms require Flash and will not display on
iPhone and iPad.
2. Click on each form one at a time and complete
3. Once you have finished a form you will find a button on the bottom of the form which says
submit
(4. The one exception is the privacy practices form; to sign that form you need to click on the
button to the left of the form that says "sign form". This will take you to another page which
will allow you to sign that form.)
5. Click on the submit button once and the form will be sent to Dr. McEvoy
6. After submitting each form, the website will return to the "patient form" page
7. Repeat this process for all three forms
8. Once Dr. McEvoy has received your forms, he will create an account for you on the
website
9. He will send you your log on ID and your password
